Dr. Daniel Joseph Mizak, 77, of Trumbull, a retired physician, passed away on Sunday, December 3, 2017 at Apple Rehab Shelton Lakes in Shelton.
He was predeceased by his beloved wife Heide. He was also predeceased by a brother-in-law, Vincent Lima. Survivors include four daughters, Andrea (Christopher) Kerin, Michelle Youd, Linda Mizak, and Heide Rose Mizak; three granddaughters, Jacqueline Kerin, Sydney Youd, and Bianca Youd; one sister, Monica Mizak; one sister-in-law, Ute Lima; one brother-in-law, Dieter (Ingrid); several nieces and nephews; many close friends; and countless patients to whom he devoted his career.
Born on January 1, 1940 in Bridgeport he was the son of the late Joseph and Rose (Turchik) Mizak. Dan grew up in Bridgeport but spent time at the family farm on Daniels Farm Road in Trumbull and with his uncle at the Fairfield County Fish and Game Protective Association club in Monroe. His active membership in the Fish and Game club lasted his entire life.
After graduation from Fairfield College Preparatory School in 1957, Dan earned his pre-medical degree at the University of Bridgeport. He took a year of special work in the department of immunology at the University of Buffalo and in 1968 earned the degree of doctor of medicine from Jefferson Medical College. At Jefferson, he was a member of the Sims Obstetric and Gynecologic society. In Philadelphia, he served his internship at Methodist Hospital and two residencies in internal medicine and cardiology at Jefferson Medical College Hospital. He also served a residency in family medicine at Bridgeport Hospital.
From 1970 to 1972, Dan served in the U.S. Army at Fort Monmouth, NJ after which time he returned with his wife and children to Connecticut, and opened his family practice office in Huntington Center in 1973, where he cared for area families until his retirement in 2012.
Dr. Mizak was known to his patients as a compassionate and caring physician. He was truly dedicated to his patients, and their very best interests were his deepest concern.
An avid sportsman, Dan was an active rifle, pistol, and shotgun shooter. While in the Army, he was a member of the Fort Monmouth Marksmanship Unit. He enjoyed hunting and fishing, and snowmobiling in New Hampshire.
Friends may greet the family on Friday, December 15 from 4:00 to 8:00 p.m. at the Spadaccino Funeral Home, 315 Monroe Turnpike in Monroe. A Mass of Christian Burial will be celebrated on Saturday, December 16, 2017 at 10:00 am at St. Lawrence Church, 505 Shelton Avenue in Huntington. Burial will be private.
The family would like to thank caregivers Pam, Aiesha, and John for their compassionate and skilled care.

Bob Dubos

Dan and I met in 1960 at the University of Bridgeport. We quickly became best of friends and shared our off time racing hydroplanes, shooting all sizes and calibers of guns and vying for girl friends. He was a night person and I was a day person which meant I would spend an hour trying to wake him up at 10 in the morning to get going. Dan was an usher in my wedding and I think I was his best man at his. We went our separate ways when he went off to Jefferson Med and I to grad school at UConn. He was my mom's physician for many years and she loved him dearly.
My wife, Pat and I planned to attend calling hours, but have been scared off by the forecasted snow. Our love to you four daughters. Losing a loved one is never easy. Dan and Heidi were great friends and we will miss them both dearly.

Dear Michele and family,
Dr. Mizak was our family doctor for my mother and father and my three daughters for many, many years. He was kind and caring and would talk with each patient and got to know all of us well, a true family doctor! What a loss it was when because of insurance we had to leave the comfort of his care. He cared for my Dad through Alzheimer's and helped us keep him at home until he passed. He surely has a special spot in Heaven and in our hearts forever. My deepest condolences.

Dear Michele and family, Your Dad was our family doctor for so many years, and we loved him so much. He did everything he could for you, was interested in your life and willingly shared his with us. He came to the house when my Dad died to spare us having to call in anyone else. You and your Mom and Dad walked my Mom to the car from your office on her last day because you knew it was the end. No one could ask for more care and compassion from any family. He served the Huntington community with such dedication. He and your Mom were a great team and now they are together again. Our prayers are with all of you and our sincere appreciation for many, many years of care and compassion. He really was one of a kind!

So sorry to hear of the passing of Dr Mizak. My mother always spoke fondly of him, he was wonderful to my grandmother when we moved her to Shelton from Georgia with advanced Alzheimer's. I remember my mother being impressed that a Dr could ride a motorcycle!! RIP Dr Mizak, you did a good job here.

My deepest sympathy on the passing of our good friend and colleague Dan Mizak. I have always been grateful to Dan for setting me up in practice which I entered in 1984. He allowed me to share his office and supported my entry into the community. I have always admired Dan for his total commitment to each of his patients. They loved and admired him. He was always committed to learning and applying the knowledge in patient care.
I will miss him greatly and will always keep a corner of my heart to his memory.
Shelton and the Griffin community has lost a champion. Dan was the ultimate patient advocate. May he Rest In Peace.
