John Sladky De Los Reyes
September 28, 1971 to July 15, 2012

John, 40, a native of San Francisco and 30 year resident of Santa Clara County passed away July 15, 2012. John is survived by his mother Janet Magat Sladky De Los Reyes and his father Jesus Lansangan De Los Reyes; his sister Jennifer and brothers Jesse and Jason; as well as numerous other family members.
A Memorial Celebration of John's life will be Saturday, July 21st at 11am at Lima Family Santa Clara Mortuary, 466 N Winchester Blvd.
