Thomas Lawrence Slezak was born on February 6, 1940 and was the son of Stanley Paul and Helen Slezak. Thomas served in the United States Navy aboard the U.S.S. Galveston during the Castro/Cuban Crisis.
He will be interred with Military Honors at Calverton National Cemetery on February 6, 2015.
